Internet Access Parental Consent Form

Please read the following carefully:

Since the Internet is a global electronic network, there can be no absolute control of its users or content. The Internet and the available resources in its global electronic network may be erroneous, out-of-date, illegal or considered offensive, controversial or sexually explicit.
Morton Grove Public Library does not monitor, and has no control over, the information accessed through the Internet and cannot be held responsible for its content.
As with other library materials, restriction of a child's access to the Internet is the responsibility of the parent or legal guardian. Parental consent is required for all children using the Internet computer in the Youth Services Department.
Child Safety on the Information Highway is an informative online brochure on the issue of children and their utilization of online resources. Parents are encouraged to work closely with their children to select Internet files which are consistent with their family's values and boundaries.

Inappropriate Use:
The computing resources offered by Morton Grove Public Library are to be used in accordance with principles of respect for the equipment and other users of the systems.

Examples of unacceptable use (some of which may have legal consequences) include, but are not limited to, the following:

Sites which contain inappropriate violent material or inappropriate sexual material

Use of the Library's Internet stations for illegal activities

Violation of computer system security

Tampering with computer hardware or software

Storage of data or files to anything other than a personal diskette

Violation of the guidelines of other networks accessed

Unauthorized use of computer accounts, codes, or ID numbers assigned to others

Violation of software license agreements and copyright laws

Violation of another user's privacy

Inappropriate use of the computer facilities of Morton Grove Public Library may result in the user losing his/her computer use privileges. Illegal acts involving library computing resources may also be subject to prosecution by local, state or federal authorities. Morton Grove Public Library staff may ask the child to exit a particular site which violates the Internet Use Guidelines in the Youth Services Department.
